To prevent the chemical spill, the tank and all other equipment used should have been checked before use.

The tank or the hose connections were obviously faulty. All equipment and machinery should be checked both before and after use and regular maintenance carried out.

The same applies to the vehicle. Regular maintenance may have prevented the initial breakdown.
